Open-for is the number that matters here, not the count. A defect closed in six hours and one closed in three days both show as closed. The three-day one took a unit out of the schedule for three days, and nothing on a board of counts says so.
Why three columns and not two. The regulation is a chain of three signatures, not a ticket that opens and closes: the driver reports the defect, a mechanic certifies the repair, and the next driver acknowledges that certification before operating the vehicle. Most fleet tools model steps 1 and 2 and drop step 3, which is exactly where the audit finding lands. A repaired unit with no driver acknowledgment is still a violation.

The inspection log and the duty log have to agree. A pre-trip inspection is on-duty-not-driving time, so it leaves a mark in both systems. When it appears in only one, something is wrong: drive time with no pre-trip means the inspection never happened, and an inspection signed during an off-duty period means it was back-filled. Neither is visible if you read either log on its own. This is the same reconciliation the IFTA module runs on odometer versus telematics miles, pointed at a different pair of sources.

Three sources, not two. The tax module reconciles odometer against telematics and the inspection module reconciles the DVIR against the duty log. Money needs a third leg: what was authorized on the work order, what was performed and certified, and what was billed. An invoice that matches the authorization but has no certified closeout is a bill for work nobody confirmed happened.

A legal driver in a legal truck can still be an illegal dispatch. Eligibility is the intersection of three things: the driver's credentials, the vehicle's registration and permits, and the jurisdictions the route actually crosses. Each one is tracked in a different place in most fleets, which is why the violation is usually found at a scale house rather than before the unit leaves.

The medical certificate is the one that catches people. A CDL can be current for another two years and the driver still cannot legally operate, because the DOT medical card behind it expired and the license downgrades. Tracking the CDL date alone leaves that invisible until an inspection finds it.

Fifteen days or forty-nine, and the notice date is what separates them. Every renewal that took over forty days was a state-issued credential. The ones the fleet controls directly closed in under a week. That is the difference between a reminder problem and a lead-time problem, and only one of them is fixed by sending notices earlier.
Issued and recorded are not the same day. A state can issue a renewal weeks before the physical document reaches the file, and until it is recorded the driver is still carrying the old one at a roadside inspection. Step four exists because that gap is where the citation happens, not step three.

Two decisions worth naming. First, permission is scoped by region as well as by role: Sarah Monroe has Edit on maintenance, but only for the 349 units in Northeast and Southeast. A role alone is not an answer in a multi-region fleet.
Second, Approve repair over $2,500 is its own capability rather than a consequence of Edit on maintenance. Approval authority is a financial control, not a workflow step, which is why Finance holds it and the Maintenance Manager who opens the work order does not.

Field mapping is a column, not a settings page. A source can be green and still be feeding you a partial picture. Toll Aggregator is connected and healthy, and 24% of its fields are unmapped, which quietly understates cost per mile. Connected and complete are different questions, so the table answers both.

Recipients are roles plus region, never named people. Naming an individual means the rule silently breaks the day they change jobs, and in a fleet that is how a critical fault ends up notifying nobody. Escalation exists for the same reason: an unacknowledged HOS violation is a citation, so it does not get to sit in one person's queue.
